I, the undersigned parent or legal guardian, acknowledge and agree to the following terms on behalf of my child(ren) (“Participant”) participating in programs offered by Dot Dot Studios / Dot Dot LLC, located in Altadena, California. For purposes of this agreement, “programs” may include sewing and creative arts classes, camps, workshops, Crafty Saturdays, parties, special events, and other activities offered by Dot Dot Studios.

Assumption of Risk

I understand and acknowledge that participation in sewing, crafting, and other creative activities involves certain known and unknown risks. Depending on the program, participants may use or be in proximity to sewing machines, sewing machine needles and moving machine parts, hand-sewing needles, pins, scissors, cutting tools, irons and other heated equipment, adhesives, craft tools, and art and craft materials. I understand that the use of sewing machines and sewing equipment carries a risk of injury, including but not limited to needle punctures or penetration injuries to fingers or hands, cuts, pinches, and other injuries resulting from contact with needles or moving parts of a sewing machine. The use of scissors, hand-sewing needles, pins, irons, adhesives, craft tools, and other equipment or materials may also result in cuts, punctures, burns, skin irritation, or other injuries.

Programs may take place indoors and/or outdoors and may involve additional ordinary risks, including trips, slips, falls, exposure to weather or outdoor conditions, contact with materials, and injuries that may arise from the actions or behavior of the Participant or other participants.

I understand that Dot Dot Studios provides age-appropriate instruction and safety guidance regarding the proper use of tools and equipment and takes reasonable precautions to provide a safe environment. However, I understand that the risks associated with these activities cannot be completely eliminated. I voluntarily assume the risks reasonably associated with my child’s participation in Dot Dot Studios programs.

Release of Liability

To the fullest extent permitted by law, I, on behalf of myself and the Participant(s), our heirs, assigns, representatives, and next of kin, hereby release and hold harmless Dot Dot Studios, Dot Dot LLC, its owner, instructors, employees, assistants, representatives, and affiliates from claims for injury, loss, liability, or damage arising out of or related to participation in Dot Dot Studios programs or presence at a Dot Dot Studios activity, except to the extent such claims cannot legally be waived or released.

I understand that Dot Dot Studios takes reasonable precautions to promote participant safety; however, participation in sewing, crafting, creative, recreational, indoor, and outdoor activities involves inherent risks that cannot be completely eliminated.

Health, Allergies & Emergency Information

I agree to provide Dot Dot Studios with accurate and current information regarding any allergies, medical conditions, dietary restrictions, or other health or safety needs that may be important to my child’s participation.

I understand that some Dot Dot Studios programs may include snacks, drinks, or refreshments. I am responsible for informing Dot Dot Studios of any known food allergies or dietary restrictions before my child participates.

I understand that certain Dot Dot Studios programs may be designated as nut-free, and I agree to follow any food and allergy guidelines communicated for the program.

In the event of an emergency, I authorize Dot Dot Studios to contact emergency medical services when reasonably necessary and to contact the parent/guardian and/or emergency contact provided during registration.

Personal Belongings

I acknowledge that personal belongings brought to Dot Dot Studios classes, camps, workshops, Crafty Saturdays, parties, or other programs are the responsibility of the parent/guardian and Participant.

Dot Dot Studios is not responsible for lost, stolen, misplaced, or damaged personal belongings.

Participant Requirements & Behavior

Participants are expected to:

• Follow all safety instructions provided by Dot Dot Studios.
• Use sewing machines, tools, equipment, and materials only as instructed.
• Behave respectfully and responsibly toward themselves, other participants, staff, studio tools, materials, and property.
• Remain within the designated classroom, studio, or activity area unless permission is given by the instructor or supervising adult.
• Participate independently with age-appropriate guidance.

If a Participant engages in behavior that is disruptive, destructive, unsafe, disrespectful, or significantly interferes with the safety or experience of other participants, I understand that I may be contacted and asked to pick up my child.In circumstances involving serious or repeated unsafe or inappropriate behavior, Dot Dot Studios reserves the right to discontinue the Participant’s enrollment or participation.

I understand that fees associated with dismissal for behavioral or safety reasons are non-refundable.

Parent-Free Classroom & Drop-Off Policy

I understand and agree that Dot Dot Studios maintains a parent-free policy for regular classes, camps, and workshops. To foster independence and maintain a focused learning environment, parents and guardians are not permitted to remain during these programs unless otherwise arranged with Dot Dot Studios.

Crafty Saturdays are designed primarily as a drop-off experience. However, we understand that every child is different. If a child needs a little extra support getting comfortable, a parent or guardian is welcome to stay when needed.

For children participating as a drop-off, parents and guardians must remain reachable using the contact information provided during registration and arrange for timely pickup at the end of the scheduled program.

Photo & Media Release

I grant Dot Dot Studios / Dot Dot LLC permission to photograph and/or record my child during classes, camps, workshops, Crafty Saturdays, parties, special events, and other Dot Dot Studios programs. I authorize Dot Dot Studios to use these photographs and/or recordings for promotional and marketing purposes, including on its website, social media, email communications, and printed materials, without compensation.

If I do not wish for my child to be photographed or recorded, I understand that I must notify Dot Dot Studios prior to my child’s participation.I understand that it is my responsibility to notify Dot Dot Studios of any changes to my photo or media preference.

Legal Authority & Agreement

I represent that I am the legal parent or guardian of the Participant(s), or that I have obtained authorization from the legal guardian to enter into this agreement on the Participant’s behalf.I affirm that I am legally competent to enter into this agreement and that I do so voluntarily.I acknowledge that I have read and understand this agreement and have had the opportunity to ask questions before agreeing to its terms.If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

Governing Law & Dispute Resolution

This agreement shall be governed by the laws of the State of California.Any dispute, claim, or controversy arising out of or relating to this agreement that cannot be resolved by mutual agreement shall be handled in accordance with applicable California law and any enforceable dispute-resolution agreement between the parties.

Parent/Guardian Acknowledgment

By signing or electronically agreeing to this waiver, I confirm that:

• I am the Participant’s parent or legal guardian, or I am authorized to act on behalf of the Participant’s legal guardian.
• I have read and understand this Participation Waiver & Release of Liability.
• I understand the nature of the activities offered by Dot Dot Studios and the risks associated with participation.
• I voluntarily consent to my child’s participation in Dot Dot Studios programs.
• I agree to the terms stated above.
