I, the undersigned parent or legal guardian, acknowledge and agree to the following terms by enrolling my child(ren) (“Participant”) in classes at Dot Dot Studios, located in Altadena, California.

Assumption of Risk

I understand and acknowledge that participation in sewing classes involves known and unknown risks, including but not limited to the use of sewing machines, needles, pins, scissors, sharp tools, and hot irons. I voluntarily assume all risks associated with my child’s participation, including risks that may arise due to the actions or behavior of other participants.

Release of Liability

I, for myself and on behalf of the Participant(s), our heirs, assigns, representatives, and next of kin, hereby release, waive, and hold harmless Dot Dot Studios, its owner, instructors, employees, and affiliates from any and all claims, injuries, losses, liabilities, or damages arising out of or related to participation in classes or presence on the premises. Participation is at my own and my child’s risk.

Dot Dot Studios takes every reasonable precaution to ensure safety; however, I understand that Dot Dot Studios cannot accept responsibility or liability for injury, loss, or damage that may occur.

Personal Belongings

I acknowledge that any personal belongings brought into the studio are the responsibility of the parent/guardian and Participant. Dot Dot Studios is not responsible for lost, stolen, or damaged items.

Student Requirements & Behavior

I acknowledge that:

• Participants must be at least 7 years old and able to work independently with guidance.
• Participants are expected to behave with respect and responsibility toward themselves, others, studio tools, and property.
• Participants are not permitted to leave the classroom at any time without first receiving permission from the instructor.

If a Participant is disruptive, destructive, unsafe, or disrespectful toward others or property, I understand that I will be contacted immediately. In such cases, the Participant may be dismissed from the program and will not be permitted to return, and I acknowledge that all class fees are 100% forfeited.

Parent-Free Classroom Policy

I understand and agree that Dot Dot Studios maintains a parent-free classroom policy. To foster independence and maintain a focused learning environment, parents and guardians are not permitted to remain in the classroom during class time.

Photo & Media Release

I grant Dot Dot Studios permission to photograph and/or record my child during classes and allow the use of these images or recordings for promotional, marketing, website, and social media purposes without compensation or limitation.

Legal Authority & Agreement

I represent that I am the legal parent or guardian of the Participant(s) enrolled, or that I have obtained authorization from the legal guardian to execute this agreement on their behalf. I affirm that I am legally competent to enter into this agreement and that I do so voluntarily and without coercion.

If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

Governing Law & Arbitration

Any dispute, claim, or controversy arising out of or relating to this Agreement that cannot be resolved by mutual agreement shall be settled exclusively through binding arbitration in accordance with the rules of the American Arbitration Association. Arbitration shall take place within Los Angeles County, California, within 25 miles of Altadena, CA.
